Credentials: I am a qualified and registered Counsellor and Supervisor with Psychotherapy &
Counselling Federation of Australia (PACFA registration number 23451). A copy of our Code of Ethics
is available upon request. I practice as a counsellor and Supervisor under Urban Sparrow
Counselling and Supervision Services.
Values: I seek to provide clients with a space to discuss and explore issues relating to their practice
and within the limits of the training undertaken as a counsellor and supervisor.
Confidentiality: In consenting to use this service, you are agreeing to have confidential records
maintained and secured with this practice. The release of information or records will only occur with
your prior permission, unless expressly related to imminent harm of self or others, whereby I have a
legal obligation to report these intentions.
Provision of Services: The provision of service will be based on mutual agreement between yourself
and Urban Sparrow Counselling and Supervision Services. I will not deliberately extend the provision
of services for my own gain. Further I will not cease the provision of services without appropriate
discussion with you, except in circumstances where the wellbeing of staff or client is, or is
potentially, jeopardised.
Your rights: You control what is discussed during your visit. If at any time you are dissatisfied with the service provided to you, I would welcome and appreciatethe opportunity to hear your concerns and feedback. If we are not able to resolve your concerns, you may refer the issue to my professional association.
Cancellation: Cancellation without notice or within 8 hours of appointment may incur a fee.
Parent/Guardian: As parent/guardian, I understand the services offered by Urban Sparrow Counselling and Supervision Services and consent to the client accessing these services. I am aware of issues pertaining to confidentiality and disclosure of client information, particularly relating to client expression of self-harm or harm to others. I understand that client sessions will not be booked without my authorisation and I may withdraw my consent for client services at any time. I agree to fund client sessions unless otherwise stated or agreed.
